Image Release

As part of your registration you have agreed that photographs, slides, movies, recordings,  video, or other media coverage of me may be taken in connection with my participation in the activity without compensation from Cabrillo and the officers, employees, and agents of same and consent to the use of photographs, pictures, slides, movies, videos, or other media coverage for any legal purpose.

If you do not consent you must provide written notice to Cabrillo Extension no fewer than 5 business days after the registration is complete.
 

Liability Release and Hold Harmless Agreement

As part of your registration for the above noted class you have requested voluntary participation to participate in the above class sponsored by Cabrillo College Extension which is hereinafter referred to as the “activity.”

As part of your registration you have acknowledged that your/minor's participation may involve risk of serious injury or death, including losses which may result not only from your own actions, inactions or negligence, but also from the actions, inactions, or activity being conducted. As part of your registration you have agreed that if you have any risk concerns, you should discuss the risks associated with your participation in the activity with Cabrillo Extension before the activity begins.

As part of your registration you/minor have acknowledged that you are in good health and have no physical conditions that prevent participation in this activity. Furthermore, you agree to use your  personal medical insurance as a primary medical coverage payment if accident or injury occurs.  

Knowing and understanding the risks involved with participation in the activity, you have voluntarily and willingly assumed responsibility for all risks and dangers associated with your participation in the activity.  You agree that you are financially responsible for any losses resulting from your actions and will indemnify Cabrillo College and the officers, directors, employees, and agents of same for any loss or damage caused by myself during this activity.

As part of your registration you have waived all claims or cause of action against Cabrillo College and the officers, directors, employees, and agents of all of same, arising out of my participation in the activity and hereby release, hold harmless, and discharge Cabrillo College and the officers, directors, employees, and agents of each of same from all liability in connection therewith except such loss or damage which was resulting to the sole negligence or willful misconduct of  Cabrillo College and its officers, employees, representatives, and volunteers, and the officers, directors, employees and agents of each of same.

As part of your registration you agree that you understand the terms used herein and their legal significance. As part of your registration this waiver and release was freely and voluntarily given with the understanding that right to legal recourse against Cabrillo College and the officers, directors, employees, and agents of each of same is knowingly given up in return for allowing your participation in the activity.

By completing your registration you have agreed to bind not only yourself but also your succession, heirs, representatives, administrators, and assigns.
